How BMI relates to GLP-1 eligibility

Wegovy, Zepbound, and Saxenda are FDA-approved for adults with BMI ≥30, or BMI ≥27 with at least one weight-related comorbidity such as hypertension, type 2 diabetes, dyslipidemia, or obstructive sleep apnea. BMI is one screening signal. Clinicians use additional criteria including waist circumference, metabolic markers, and medical history.

Limitations of BMI

BMI does not distinguish between muscle and fat mass, so it can misclassify athletes and people with high lean muscle as "overweight." It also doesn't reflect fat distribution (visceral vs subcutaneous), which matters more for metabolic risk than total weight. Treat BMI as a starting screen, not a diagnosis.
