The list of drugs an insurance plan covers, organized into cost tiers.
Each plan's formulary determines whether a GLP-1 is covered and at what copay tier. The same molecule may be on-formulary for diabetes (Ozempic) but excluded for weight loss (Wegovy). Always confirm the weight-management indication specifically.
