Self-pay price log

Every figure below was read from the publisher named on its row on 2026-09-16.

A month means a different quantity in each channel — a box of four Wegovy pens, one Ozempic pen, a bottle of 30 tablets, 28 days of Zepbound. The rows are records, not a ranking, and nothing here is added up or averaged across channels.

Read this before you accept a quote

Four places where the published numbers disagree with each other. Each one is a way to be quoted one price and charged another.

The $199 introductory price has two different expiry dates
Novo Nordisk's November 17, 2025 press release set the two-fill $199 introductory offer to expire March 31, 2026. The April 2026 NovoCare price guide runs it through December 31, 2026. If you were quoted $199, confirm the end date against the current price guide, because the next fill after it lapses is $349, or $399 for the 7.2 mg HD pen. Source
NovoCare's '$25 per month' is a copay, not a self-pay price
The $25 figure on NovoCare's pharmacy landing page applies to patients with commercial insurance, caps at $100 of savings per month, requires local pharmacy pickup with the savings offer, and excludes government beneficiaries. Nobody paying cash pays $25. The cash prices are $349 for the Wegovy pen and $399 for the 7.2 mg HD pen. Source
The 4 mg Wegovy tablet is listed at two prices
The April 2026 NovoCare price guide lists the 4 mg tablet at $199 per month as the standard price. The NovoCare Pharmacy terms page lists a limited-time $149 for the same strength, for prescriptions written and received by August 31, 2026 and shipped by September 30, 2026. Which one you pay depends on the dates on your prescription. Source
TrumpRx's $199 Wegovy pen price matches the introductory offer, not the standard one
TrumpRx shows $199 per month for the Wegovy pen against a $1,349 comparison price, with no dose strength, no effective date and no eligibility conditions. That $199 is Novo's two-month introductory price for new patients, not the $349 standard self-pay price, and the site does not say which applies to you. Source

Retailers that publish no price

We record the absence rather than fill it in. A price that a retailer does not print is not a price.

Costco
Costco publishes no public cash price for any GLP-1. The pharmacy page returns HTTP 403 to automated access, /pharmacy/glp-1 returns 404, and pharmacy.costco.com/glp-1 returns an empty shell with no GLP-1 text and no dollar figures. Costco's GLP-1 offering is a distribution arrangement for Novo Nordisk's self-pay price rather than a Costco-set price: Novo's November 17, 2025 release names Costco as one of its partner organizations. Treat any Costco-specific figure as unverified.Checked 2026-09-16: https://www.costco.com/pharmacy.html · https://www.costco.com/pharmacy/glp-1 · https://pharmacy.costco.com/glp-1
Cost Plus Drugs
No published price for Wegovy, Ozempic, Zepbound or Mounjaro could be verified. The site sits behind Cloudflare and returns HTTP 403 to all automated access. Search-result metadata indicates the catalog carries generic liraglutide, a different GLP-1, but not branded semaglutide or tirzepatide — Cost Plus does not sell Ozempic, Wegovy, Zepbound or Mounjaro. The liraglutide price was not verified. Circulating '$60-$130 per month semaglutide' figures refer to compounded product from telehealth vendors, not Cost Plus, and are not sourced.Checked 2026-09-16: https://www.costplusdrugs.com/medications/index.html · https://www.costplusdrugs.com/medications/search/?q=semaglutide · https://www.costplusdrugs.com/faq/

Walmart sets no independent cash price. It is a pick-up point for LillyDirect self-pay pricing, at the same price as online. TrumpRx publishes headline monthly prices but no dose strengths, no effective dates and no eligibility conditions for any GLP-1. Claims we could not verify

Costco currently sells Wegovy and Ozempic at $349 per month
Not verified. It follows logically from Novo's November 17, 2025 cut applying to partner channels including Costco, but Costco publishes no price and no primary source states a current Costco figure. We do not quote a Costco price.
Lilly extended the $299/$399/$449 tier to the Zepbound KwikPen on February 23, 2026, making vials and pens the same price
Date not verified on a primary or reputable dated source. The price parity itself is verified — LillyDirect showed identical $299, $399 and $449 pricing for the KwikPen and the vials as of September 16, 2026 — but the February 23, 2026 date comes only from content-farm search snippets.

Lilly issued a media statement with this title, but the release body could not be retrieved (investor.lilly.com returns HTTP 403). Amazon Pharmacy's March 9, 2026 release independently confirms the KwikPen at a $299 starting cash price outside LillyDirect.

Charity and manufacturer assistance: what is actually open

3 of the 10 programs below were closed to new applicants on that date, including every charity fund that pays toward a weight-loss GLP-1. TotalAssist (Patient Advocate Foundation; merged with PAN Foundation)

    Obesity fund

The catch: This is the one charity fund built for weight-loss drugs, and it is closed. Its own rules also require health insurance that already covers the expense, so a cash-pay patient would not qualify even when it is open.

  • Closed

TotalAssist (Patient Advocate Foundation; merged with PAN Foundation)

    Type 2 diabetes (T2DM) fund

The catch: Closed, and it was never open to everyone: this fund requires government-insured coverage, so commercially insured and uninsured patients are ineligible. The companion type 2 diabetes health equity fund is closed as well.

    Income limit as stated
Have an income at or less than 500% of the Federal Poverty Level, adjusted for your regional Cost of Living Index (COLI)

    TotalAssist (program-level rules)

    Program-wide eligibility (nearly 150 funds)

The catch: The program runs, but you can only apply to a fund that is open, and both funds that reach GLP-1s — obesity and type 2 diabetes — were closed on the date checked. A closed fund offers a notification sign-up, not a waitlist queue.

    Income limit as stated
Have a household income of 500% or less than the Federal Poverty Level (FPL) guidelines, adjusted by the Cost-of-Living Index (COLI) for where the patient lives — with the exception: “Household income of 300% or less than the Federal Poverty Level (only our obesity fund)”

    HealthWell Foundation

    Type 2 Diabetes fund

The catch: HealthWell has no obesity or weight-management fund at all, so a weight-loss prescription has no route here. The type 2 diabetes fund is the only one that reaches a GLP-1, and it is closed: “This fund is temporarily closed to new patients due to lack of sufficient funding.”

    Income limit as stated
300–500% of the Federal Poverty Level (fund page); program-wide, “up to 400 or 500 percent of the Federal Poverty Level (somewhat more in cities where the cost of living is higher)”

Novo Nordisk Patient Assistance Program (PAP)

Manufacturer patient assistance program (free medicine)

The catch: Ozempic is the only GLP-1 on the product list. Wegovy and Saxenda are not on it, so there is no free-medicine route to a branded obesity GLP-1 from Novo Nordisk. For Ozempic the program takes uninsured patients only, at or below 200% of the federal poverty level.

    Income limit as stated
Ozempic (uninsured): “Total household income must be at or below 200% of the federal poverty level”; insulins and all other products: “Total household income must be at or below 400% of the federal poverty level”
    Insurance requirement
Must have Medicare or no insurance; private or commercial insurance makes a patient ineligible. For Ozempic, uninsured patients only — Medicare Part D patients are no longer eligible for Ozempic through PAP.

    Lilly Cares Foundation

    Lilly Cares Patient Assistance Program

The catch: The only GLP-1 offered is Trulicity, a type 2 diabetes drug. Zepbound and Mounjaro appear in no medication group and on no Available Medications page, so tirzepatide has no free-medicine route here. Private commercial insurance disqualifies.

    Income limit as stated
    Group 1 Medications (at or below 300% FPL); Group 2 Medications (at or below 400% FPL); Group 3 & Group 4 Medications (at or below 500% FPL) — 2026 FPL guidelines. Household of 1/2/3/4 for Group 1: $47,880 / $64,920 / $81,960 / $99,000.
    Insurance requirement
For Group 1, 2 and 3 medications, which includes Trulicity: “1) You have no insurance, or 2) you have Medicare Part D (not applicable to infused medications), or 3) you have Medicare Part B but have no supplemental or secondary insurance”
    Award
Free medication; enrollment generally lasts 12 months or to the end of the enrollment period

    Patient Advocate Foundation

Case Management Services & MedCareLine; Financial Aid Funds

The catch: Case management is advocacy, not money: it does not pay for medication. It is worth a call for an insurance denial or appeal. The grant arm is TotalAssist, whose obesity and type 2 diabetes funds are closed.

    Income limit as stated
No income threshold is published for case management; TotalAssist income limits apply to grants
    Insurance requirement
No insurance requirement is stated for advocacy; case management is offered to patients “facing a chronic, life-threatening or debilitating diagnosis”
    Award
Case management carries no grant; TotalAssist grants vary by fund

    Case Management (800) 532-5274 (Mon–Fri 8:30am–5pm ET); TotalAssist (866) 512-3861; Financial Aid Funds (855) 824-7941

    NeedyMeds

Information directory and free drug discount card

The catch: NeedyMeds awards no money. It lists other people's programs and issues a discount card — and a discount card is not insurance, which matters because HealthWell states “Drug discount cards do not qualify as insurance.” for its own eligibility.

    Income limit as stated
None for the discount card: “Can be used by anyone — no financial, age, or residency restrictions”
    Insurance requirement
None; the card is usable with or without insurance
    Award
No grant. The card is advertised as “Save up to 80%” on prescriptions.

  • Directory only

    RxAssist

    Directory of patient assistance programs

The catch: A lookup tool, not a funder: no money, no grants, no application, and no patient helpline published on the site. Use it to find manufacturer programs, not to pay for a prescription.

    Income limit as stated
None — RxAssist awards nothing; income rules belong to the programs it lists
    Insurance requirement
    None stated
    Award
    No award; directory only
    Phone
No patient helpline published on the site

    AZ&Me Prescription Savings Program (AstraZeneca)

    Manufacturer patient assistance program

The catch: No GLP-1 for obesity is offered by this program, and its two diabetes drugs stopped taking new patients: “Effective May 1, 2026, the AZ&Me program will no longer accept new patients for FARXIGA and XIGDUO XR”. Any commercial insurance disqualifies.

    Income limit as stated
Annual adjusted gross income must be at or below 300% of the Federal Poverty Level (FPL) for Primary and Specialty Medications; 500% of FPL for Rare Disease Medications
    Insurance requirement
“Not have any commercial (private or employer-sponsored) insurance or government insurance other than Medicare”
    Award
Medication at no cost, up to a 90-day supply per fill
    Phone
    1-800-292-6363 (Mon–Fri 9:00am–6:00pm ET)

Manufacturer copay cards. The Zepbound, Mounjaro, Wegovy and Ozempic savings cards all exclude patients in a government program. The Wegovy terms put it plainly: “Patient is not eligible if he/she is enrolled in any federal or state health care program with prescription drug coverage, such as Medicaid, Medicare, VA, DOD, TRICARE, or any similar federal or state health care program”. If you are on Medicare or Medicaid, the $25 card price is not available to you, whatever the ad says.

Compounded GLP-1s. No program reviewed covers compounded semaglutide or tirzepatide. TotalAssist limits approved medications to products that are “FDA-approved, listed in official compendia, and/or listed in published evidence-based or clinical guidelines”. Manufacturer programs supply only their own branded products.

Telehealth prescriptions. No program reviewed publishes a telehealth exclusion on its own site. Where a prescription was written is not something these programs ask about in their published rules.
