Manufacturer-direct vs. warehouse and retail cash prices for GLP-1s (2026)
Costco, Walmart and Amazon do not set their own GLP-1 cash prices — they pass through what Novo Nordisk and Eli Lilly publish. Here is what each channel actually prints, what it refuses to print, and the four places the numbers contradict each other.

The warehouse clubs are not setting these prices
The cash market for branded GLP-1s in 2026 has one price-setter per drug: Novo Nordisk for Wegovy and Ozempic, Eli Lilly for Zepbound and Mounjaro. The retail names attached to GLP-1 cash pricing are distribution points for those prices, not competitors to them. That changes what is worth shopping for — pickup convenience and stock, rather than a lower number.
**Walmart** launched the first retail pick-up option for LillyDirect self-pay Zepbound vials, announced October 29, 2025 and available from mid-November, at $349 for the 2.5 mg vial and $499 for the 5 through 15 mg vials. Lilly cut those prices on December 1, 2025, so both launch figures are stale. Walmart publishes no current GLP-1 price page, and its own release describes the arrangement as pick-up at the same price as LillyDirect online.
**Costco publishes no price for any GLP-1.** Its pharmacy landing page returns HTTP 403 to automated access, its GLP-1 URL returns 404, and the pharmacy subdomain returns an empty shell with no GLP-1 text and no dollar figures. What exists is an announcement: Novo Nordisk said in early October 2025 that Costco pharmacies would sell a four-week supply at $499 out of pocket at more than 600 US locations, reported by NBC News on October 3, 2025. Novo's November 17, 2025 release then names Costco among the partner organizations covered by the cut to $349. A reader can reasonably expect Novo's price at a Costco counter, but Costco has not printed a number, so we do not quote one.
**Cost Plus Drugs does not sell branded GLP-1s at all.** No Ozempic, Wegovy, Zepbound or Mounjaro. Catalog metadata indicates generic liraglutide, which is a different GLP-1, and we could not verify its price because the site returns HTTP 403 to automated access.
**Amazon Pharmacy publishes 'starting at' figures only.** Its March 9, 2026 release quotes a cash price starting at $299 per month for the Zepbound 2.5 mg starter KwikPen, and its January 9, 2026 release quotes the Wegovy pill starting at $149 per month. Neither gives a per-dose list, the pricing page shows no GLP-1 prices without sign-in, and the product pages render no price to signed-out visitors. A starter-dose figure is the weakest possible guide to what you will pay at a maintenance dose.
What Novo Nordisk publishes
The Wegovy pen is $349 per month for the 0.25 through 2.4 mg doses, effective November 17, 2025, where a month is one box of four pens. The 7.2 mg HD pen is $399, and the price guide states no effective date for that tier. New patients pay $199 for each of the first two fills of 0.25 mg and 0.5 mg, through December 31, 2026.
The Wegovy pill is priced by dose: $149 for the 1.5 mg tablet, $199 for 4 mg, and $299 for the 9 mg and 25 mg tablets, where a month is one bottle of 30 tablets.
Ozempic pens are $349 for 0.25, 0.5 and 1 mg — but the 2 mg pen was explicitly excluded from the November 2025 cut and remains $499 per month. If you titrate to 2 mg, your monthly cost changes. Oral Ozempic runs $149, $199 and $299 by dose on the same 30-tablet basis.
One condition applies across all of it: the patient certifies they will not seek reimbursement from any insurer, commercial or government. For Ozempic specifically, government beneficiaries are excluded from the savings offer, and a patient holding both commercial and government-funded coverage counts as government-insured. FEHB, Health Exchange and state employee plans are not treated as government programs. Novo Nordisk states it may modify or cancel the offer at any time.
What Eli Lilly publishes
Zepbound self-pay through LillyDirect is $299 for 2.5 mg and $399 for 5 mg, effective December 1, 2025. A month here means 28 days and up to one single-patient-use KwikPen, or four single-dose vials.
The 7.5, 10, 12.5 and 15 mg strengths are listed at $449 — conditionally. That price applies only when the prescription is refilled within 45 days of the previous fill under the Zepbound Journey Program. If the window lapses, the regular prices apply: $499 for 7.5 mg and $699 for 10, 12.5 and 15 mg. This is the single largest gap between a quoted price and a paid price anywhere in this data.
Three more conditions sit on the Lilly side: pen needles are a separate purchase, the card covers a maximum of 11 fills per calendar year, and the card expires December 31, 2026. The single-dose auto-injector pen is not offered on self-pay terms at all — only the single-patient-use KwikPen and the single-dose vials carry this pricing.
Mounjaro is listed as starting at $499 per month for every strength from 2.5 to 15 mg, with no per-dose differentiation and no effective date published. Lilly's November 6, 2025 agreement with the US government said Mounjaro would be added to LillyDirect at 50 to 60 percent off list without naming a figure, so there is no dated change record to build for Mounjaro.
Four places the published numbers contradict each other
**The $199 introductory price has two expiry dates.** Novo's November 17, 2025 press release set the two-fill offer to expire March 31, 2026. The April 2026 NovoCare price guide runs it through December 31, 2026. Confirm the end date against the current price guide before you plan around it: the fill after it lapses is $349, or $399 for the 7.2 mg HD pen.
**NovoCare's '$25 per month' is a copay, not a cash price.** It applies to patients with commercial insurance, caps at $100 of savings per month, requires local pharmacy pickup with the savings offer, and excludes government beneficiaries. Nobody paying cash pays $25. It appears on the pharmacy landing page close enough to the self-pay tiers to be misread as one.
**The 4 mg Wegovy tablet is listed at two prices.** The April 2026 price guide lists $199 as standard. The NovoCare Pharmacy terms page lists a limited-time $149 for the same strength, for prescriptions written and received by August 31, 2026 and shipped by September 30, 2026. Which one you pay turns on the dates on your prescription, not on anything you choose at checkout.
**TrumpRx's $199 for the Wegovy pen is the introductory price, not the standard one.** The site shows $199 per month against a $1,349 comparison price, with no dose strength, no effective date and no eligibility conditions for any GLP-1 it lists. That figure matches Novo's two-month offer for new patients rather than the $349 standard self-pay price, and the site does not say which applies to you. Mounjaro is not listed there at all.
If you are on Medicare, the cash prices are not your comparison
The Medicare GLP-1 Bridge launched July 1, 2026 and runs through December 31, 2027. The beneficiary copay is $50 per monthly supply, and participating manufacturers supply eligible drugs at a net price of $245 per monthly supply. That $245 is a manufacturer figure, not something a patient pays.
The eligibility rules are narrow in a way the headline number hides. You must be enrolled in Part D and prescribed an eligible GLP-1 for weight management, and you must not already be eligible for GLP-1 coverage under the standard Part D benefit. A beneficiary with a Part D-covered indication — type 2 diabetes, obstructive sleep apnea, cardiovascular risk reduction, or MASH with liver fibrosis — is not eligible for the Bridge. Neither is anyone who received a GLP-1 through Part D in CY2026.
Two accounting details matter if you are tracking your drug spending: the Part D deductible does not apply to the $50, the $50 does not count toward your true out-of-pocket threshold, and no low-income subsidy applies to it. Amazon Pharmacy handles eligibility verification, prior authorization and billing for Bridge fills it dispenses.
Five questions that settle what you will actually pay
**Ask which tier you were quoted.** Introductory or standard, and the exact date the introductory tier ends. The difference on a Wegovy pen is $199 versus $349, and on the HD pen $199 versus $399.
**For Zepbound, ask about the 45-day window.** Confirm whether the $449 you were quoted assumes an on-time refill, and what the price becomes if you miss it. At 10 mg and above, that is $449 versus $699.
**Ask what a month buys.** Four Wegovy pens, one Ozempic pen, a 30-tablet bottle, or 28 days of Zepbound are four different quantities. Prices from different channels are not directly comparable until you have matched the unit.
**Ask whether the number is a cash price or a copay.** A $25 or $50 figure almost always assumes insurance or a government program behind it.
**Do not plan around a retailer price nobody published.** If a figure is attributed to Costco, ask where it is printed. As of September 16, 2026 it is not printed anywhere on Costco's own site.

People also ask
Does Costco have a cheaper cash price for Wegovy or Ozempic?
Costco publishes no cash price for any GLP-1. Its pharmacy landing page returns HTTP 403 to automated access, its GLP-1 URL returns 404, and the pharmacy subdomain renders an empty page with no dollar figures. What is documented is that Novo Nordisk announced in October 2025 that Costco pharmacies would sell a four-week supply at the then-current $499 self-pay price, and that Novo's November 17, 2025 release names Costco as one of the partner channels covered by the cut to $349. That makes Costco a distribution point for Novo's price rather than a retailer setting its own, and any specific Costco figure you see quoted is inferred rather than published.
Is the LillyDirect price for Zepbound the same at Walmart?
Yes, and that is the point of the arrangement. Walmart is a pick-up location for LillyDirect self-pay vials at the same price as the LillyDirect website; Walmart sets no independent cash price and publishes no current GLP-1 price page. Its only dated figures are in the October 29, 2025 launch release — $349 for the 2.5 mg vial and $499 for 5-15 mg — and Lilly superseded both on December 1, 2025.
Why is my Zepbound price higher than the $449 I was quoted?
The $449 price for the 7.5, 10, 12.5 and 15 mg strengths is conditional on refilling within 45 days of your previous fill under the Zepbound Journey Program. If the window lapses, the regular price applies: $499 for 7.5 mg and $699 for 10, 12.5 and 15 mg. Two other conditions can change what you pay: the savings card covers a maximum of 11 fills per calendar year and expires December 31, 2026, and pen needles are a separate purchase.
Does Cost Plus Drugs sell Wegovy or Zepbound?
No. Cost Plus Drugs does not carry branded semaglutide or tirzepatide — no Ozempic, Wegovy, Zepbound or Mounjaro. Its catalog metadata indicates generic liraglutide, a different GLP-1, and we could not verify a price for it because the site returns HTTP 403 to automated access. The '$60 to $130 per month semaglutide' figures that circulate alongside the Cost Plus name refer to compounded product sold by telehealth vendors, not to Cost Plus.
