PDE5 inhibitors and nitrates: the one combination that's genuinely dangerous
Most ED medication 'interactions' aren't dangerous. One is — nitrates. Understanding why, what counts as a nitrate, how long you have to wait, and what to do if you've taken both is genuinely life-saving information.

Why this combination is genuinely dangerous
PDE5 inhibitors (sildenafil/Viagra, tadalafil/Cialis, vardenafil, avanafil) increase nitric oxide signaling, causing vasodilation. Nitrates do the same thing via a different pathway. Combined, the vasodilation can be massive — blood pressure can drop catastrophically, causing fainting, heart attack, stroke, or death.
This is not a 'be careful' interaction. It's a 'do not combine under any circumstances' interaction. Emergency departments and cardiologists explicitly screen for PDE5 use before administering nitrates because the consequences are severe and avoidable.
What counts as a nitrate
Prescription nitrates (most common in cardiac patients): nitroglycerin (sublingual, spray, ointment, patch), isosorbide mononitrate, isosorbide dinitrate.
Emergency room nitrate: nitroglycerin IV for chest pain or hypertensive crisis.
Recreational nitrites ('poppers'): amyl nitrite, butyl nitrite, isobutyl nitrite. Same vasodilation mechanism, same contraindication. This is the scenario that catches people off guard most.
Some 'natural' supplements marketed for sexual or cardiovascular health may contain nitrates or have similar effects — read labels carefully.
Wait times after PDE5 use
Sildenafil (Viagra): 24 hours minimum before any nitrate. Sildenafil's half-life is ~4 hours; 24h represents ~6 half-lives = essentially complete clearance.
Tadalafil (Cialis): 48 hours minimum before any nitrate. Tadalafil's longer half-life (~17.5 hours) means longer washout. Daily-dose tadalafil (2.5-5mg) means you should not be taking nitrates concurrently at all.
Vardenafil and avanafil: 24 hours minimum.
If you've already taken both
Call 911 or get to an emergency department immediately if you experience: severe dizziness or fainting, chest pain, shortness of breath, severe headache, vision changes, profound weakness. Don't drive yourself.
Tell EMS, ER staff, and any treating clinician about both medications. They need to know to manage the hypotension correctly — standard hypotension treatment can fail if PDE5 is on board, requiring different agents.
Don't assume 'I feel okay' means you're fine. The hypotension can be delayed; effects can hit minutes to hours later. Stay where you can be monitored.
What if you have heart disease and use PDE5 inhibitors?
Many men with cardiovascular disease can safely use PDE5 inhibitors — but the medications they take for the heart matter. If you're on nitrates for chronic angina or have nitroglycerin as a rescue, PDE5 inhibitors are not safe for you. Discuss alternatives with your cardiologist.
If you're on cardiovascular medications other than nitrates (beta blockers, ACE inhibitors, calcium channel blockers, statins, aspirin), PDE5 inhibitors can usually be used with cardiology coordination. Alpha-blockers (used for BPH) require dosing separation but aren't a hard contraindication.
