FDA Boxed Warning
Risk of thyroid C-cell tumors (class effect). Orforglipron is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Why is Orforglipron prescribed?
Orforglipron is Eli Lilly's once-daily oral GLP-1 receptor agonist, the first non-peptide GLP-1 to reach the US market. Unlike semaglutide (which requires either injection or a tightly-dosed oral peptide), orforglipron is a small molecule that survives digestion, so it can be taken as a simple daily pill without the strict fasting-and-water rules that oral semaglutide (Rybelsus) demands. How does Orforglipron work?

In case of emergency or overdose
Orforglipron is a non-peptide oral GLP-1; specific overdose data is limited. As with the injectable GLP-1s there is no specific antidote, and the same general approach applies: call US Poison Control at 1-800-222-1222 immediately, or go to the ER for severe hypoglycemia, persistent vomiting, dehydration, or altered mental status. The shorter daily-pill half-life means recovery should be faster than the weekly injectables, but supportive care is still required.
