FDA Boxed Warning
Risk of thyroid C-cell tumors (class effect). Orforglipron is contraindicated in patients with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Why is Foundayo prescribed?
Orforglipron (brand name Foundayo) is Eli Lilly's once-daily oral non-peptide GLP-1 receptor agonist, FDA-approved April 1, 2026 for chronic weight management. Unlike semaglutide (which requires either injection or a tightly-dosed oral peptide), orforglipron is a small molecule that survives digestion, so it can be taken as a simple daily pill without the strict fasting-and-water rules that oral semaglutide (Rybelsus) demands. It can be taken with or without food, and LillyDirect self-pay starts at $149/month. There are no compounded versions, because it is a novel patented molecule.
How does Foundayo work?
Same target as injectable GLP-1s — the GLP-1 receptor — but a non-peptide small-molecule structure. Activates GLP-1 signaling to slow gastric emptying, suppress appetite, and improve glycemic control. The oral, digestion-stable chemistry is the breakthrough: it expands GLP-1 therapy to patients who refuse injections.
Who qualifies for Foundayo?
Adults with obesity, or overweight with at least one weight-related condition, for chronic weight management. Not approved for type 2 diabetes. Especially relevant for needle-averse patients who declined injectable GLP-1s. Same thyroid C-cell contraindication class as other GLP-1s.
How should Foundayo be used?
Once-daily oral tablet, titrated upward over several weeks to manage GI side effects. Taken with or without food, at any time of day (unlike Rybelsus, which requires an empty stomach).
What should I do if I forget a dose?
Once-daily dosing. If a dose is missed, take it the same day if remembered; skip if it is nearly the next day. Do not double up.

What side effects can Foundayo cause?
GI side effects typical of the GLP-1 class: nausea, diarrhea, constipation, vomiting, especially during titration. Trial tolerability broadly comparable to injectable GLP-1s.
What interactions should clinicians watch for?
As a GLP-1, orforglipron can affect absorption of co-administered oral drugs via slowed gastric emptying. Monitor narrow-therapeutic-index oral medications. Combined hypoglycemia risk with insulin and sulfonylureas in T2D patients.
What special precautions should I follow?
Pregnancy & lactation
Not recommended in pregnancy (GLP-1 class). Discontinue before planned conception; discuss timing with your prescriber.
Alcohol
No formal contraindication. GLP-1 GI effects plus alcohol can amplify nausea. Moderate intake, avoid during titration.
What does Foundayo cost?
LillyDirect self-pay: $149/month for 0.8 mg and 2.5 mg, $199 for 5.5 mg, and $299 for 9 mg, 14.5 mg, and 17.2 mg. The 2.5 mg and 5.5 mg prices are introductory through December 31, 2026; the 14.5 mg and 17.2 mg price requires a refill within 45 days. With commercial insurance that covers it, the Foundayo savings card can lower the cost to as little as $25/month (savings capped at $100/month, through December 31, 2026). Medicare GLP-1 Bridge: $50/month from July 1, 2026 to December 31, 2027.
In case of emergency or overdose
Orforglipron is a non-peptide oral GLP-1; specific overdose data is limited. As with the injectable GLP-1s there is no specific antidote, and the same general approach applies: call US Poison Control at 1-800-222-1222 immediately, or go to the ER for severe hypoglycemia, persistent vomiting, dehydration, or altered mental status. The shorter daily-pill half-life means recovery should be faster than the weekly injectables, but supportive care is still required.
How should Foundayo be stored?
Store oral tablets at room temperature per the product label. Keep in original packaging away from moisture.
