What Blue Cross Blue Shield specifically requires
- FEP Blue (effective July 1, 2026): age 18 or older
- BMI of 30 or more, or 27 or more with established cardiovascular disease or a weight-related condition (e.g., type 2 diabetes, dyslipidemia, hypertension)
- Inadequate response, intolerance or contraindication to at least two oral weight-loss drugs (e.g., phentermine, benzphetamine, diethylpropion, Qsymia)
- Participation in a comprehensive weight management program (e.g., Teladoc)
- No use alongside another GLP-1 or another prior-authorized weight-loss drug
- First approval: 6 months; renewal: 12 months with a 5% BMI reduction achieved or maintained
