Does Kaiser Permanente cover Zepbound?

In Kaiser Permanente Northwest, Zepbound is non-formulary. On top of the Wegovy requirements, it needs a failed trial of semaglutide (Ozempic or Wegovy) of at least 6 months and a bariatric medicine chart review. In California, GLP-1s have not been covered for weight loss alone since January 2025.
What Kaiser Permanente specifically requires
- Northwest: plan includes a benefit for weight-loss medications
- BMI of 30 or more, or 27 or more with a documented weight-related condition
- Failed a 3-month trial of at least two cheaper weight-loss drugs (or cannot take any of them)
- Then failed semaglutide (Ozempic or Wegovy) for at least 6 months, followed by a bariatric medicine chart review, or cannot take semaglutide
- Approval: 12 months
