Semaglutide does not chemically reduce the effectiveness of combined oral contraceptives — clinical pharmacokinetic studies show no meaningful interaction. The real risk is indirect: semaglutide commonly causes nausea, vomiting, and diarrhea, especially in the first weeks and after each dose increase. If you vomit within about 3 hours of taking an oral contraceptive pill, or have diarrhea lasting more than 24 hours, the pill may not be fully absorbed, which can lower its protection. Tirzepatide (Mounjaro, Zepbound) goes further: the manufacturer specifically advises using a non-oral contraceptive or adding a barrier method for 4 weeks after starting and after each dose escalation, because tirzepatide's effect on gastric emptying can reduce oral contraceptive absorption. Practical guidance: if you're on the pill and experiencing GI side effects, use a backup barrier method during titration weeks, or talk to your prescriber about a non-oral option (IUD, implant, injection, patch, ring).
