Why is Compounded semaglutide prescribed?

Compounded semaglutide is a custom-made version of the active drug, produced by 503A or 503B pharmacies. It is NOT FDA-approved. During the 2022-2024 semaglutide shortage, compounding pharmacies legally produced versions for routine prescribing. The shortage was resolved in late 2024, narrowing legal compounding to specific clinical needs. As of April 30, 2026 the FDA went further: it proposed permanently removing semaglutide from the 503B bulk-compounding list, issued 30 warning letters, and opened a comment period through June 29, 2026. Large platforms (including Hims, which exited in February 2026) have already pulled compounded semaglutide. If you are on a compounded version, plan a transition to branded Wegovy/Ozempic or a manufacturer direct-pay channel.

How does Compounded semaglutide work?

What side effects can Compounded semaglutide cause?

What interactions should clinicians watch for?

Same active ingredient (semaglutide), same interaction profile as Wegovy/Ozempic: insulin and sulfonylureas (hypoglycemia risk), oral contraceptives (effectiveness may reduce in patients with severe GI symptoms), warfarin (INR monitoring). Additional concern with compounded formulations: any added B-vitamins or other adjuvants may have their own interactions; disclose your specific compounded formulation to your prescriber.

What special precautions should I follow?

Pregnancy & lactation

Not recommended during pregnancy or breastfeeding (same semaglutide pregnancy profile as Wegovy). Discontinue at least 2 months before planned pregnancy due to long half-life. If you become pregnant on compounded semaglutide, stop the medication and contact your prescriber promptly. The 2026 narrowing of compounding legality means many patients have already transitioned to branded — this is an additional reason if planning pregnancy.

Alcohol

Same considerations as branded semaglutide: slowed gastric emptying changes alcohol absorption, severe GI side effects amplified, hangovers can be unusually severe. Moderate amounts on non-titration weeks if you drink. Note that compounded formulations may include additional ingredients (B12, glycine) with their own profiles — review your specific vial label.

What does Compounded semaglutide cost?
