What is Compounded semaglutide?
Compounded semaglutide is a custom-made version of the active drug, produced by 503A or 503B pharmacies. It is NOT FDA-approved. During the 2022-2024 semaglutide shortage, compounding pharmacies legally produced versions for routine prescribing. The shortage was resolved in late 2024, narrowing legal compounding to specific clinical needs. As of April 30, 2026 the FDA went further: it proposed permanently removing semaglutide from the 503B bulk-compounding list, issued 30 warning letters, and opened a comment period through June 29, 2026. Large platforms (including Hims, which exited in February 2026) have already pulled compounded semaglutide. If you are on a compounded version, plan a transition to branded Wegovy/Ozempic or a manufacturer direct-pay channel.
How does Compounded semaglutide work?
Pharmacologically identical to branded semaglutide. Same GLP-1 receptor agonist mechanism.
Who can take Compounded semaglutide?
Same eligibility as branded semaglutide. Telehealth platforms that still prescribe compounded versions typically document a clinically justified modification (different concentration, with added B12, etc.) to remain within 503A rules.
How is Compounded semaglutide taken?
Compounded semaglutide weight loss over time (trial average)
- Week 42%
- Week 126%
- Week 2010%
- Week 4013%
- Week 6815%
Mean total body-weight reduction at the 2.4 mg weekly maintenance dose, from STEP 1 (NCT03548935). Individual results vary widely; ~10% of trial participants lost less than 5%.
How much does Compounded semaglutide cost?
$200-500/month — significantly cheaper than branded ($1,350/month). Quality varies by pharmacy; ask which 503A/B facility the compound comes from before paying.
What side effects can Compounded semaglutide cause?
Same as branded semaglutide. Additional risk: variability in purity and concentration between pharmacies. Choose telehealth providers that name their compounding pharmacy.
Compounded semaglutide side-effect timeline: when to expect what
- Weeks 1-40.25 mg starter
Mild nausea begins for many; occasional constipation. Side effects are usually manageable at this dose. The starter dose is about tolerance, not weight loss.
- Weeks 5-12Dose escalation
Each step up (0.5 → 1.0 → 1.7 mg) can bring a fresh wave of nausea for a few days, then settle. This is the hardest stretch. Eating smaller, lower-fat meals helps.
- Weeks 13-20Approaching maintenance
Most people adapt; nausea becomes intermittent rather than constant. Constipation may persist — fiber and hydration matter.
- Week 20+2.4 mg maintenance
Side effects are typically mild and occasional at steady state. If severe symptoms persist past this point, talk to your prescriber about slowing the titration or adjusting the dose.
Typical timing from FDA labels and trial data. Individual experience varies; slower titration eases side effects for many patients.
What if I miss a dose of Compounded semaglutide?
Compounded semaglutide is typically dosed weekly (matching branded protocols). Missed-dose guidance follows the same pattern as branded Wegovy: take within 5 days of scheduled day, then resume; if more than 5 days late, skip. Do not double up. Confirm with your dispensing pharmacy if your specific formulation has different timing.
