How to vet any weight loss telehealth program
Before you pay, get a clear answer to each of these. If a provider makes any of them hard to find, that is itself an answer.
- Total all-in cost. Platform or membership fee plus medication plus shipping — the real monthly number, not the teaser. And is it the same at every dose, or does it climb as you titrate up?
- Brand-name vs compounded. An FDA-approved finished drug (Wegovy, Zepbound), or a pharmacy-compounded formulation that is not FDA-approved? After the 2026 shortage resolution, compounded GLP-1s are restricted to documented individual need.
- Named pharmacy + certification. Does it name its dispensing or 503A pharmacy and hold LegitScript certification? Their absence is a flag, not proof of a problem.
- A real prescriber. A licensed, board-certified clinician doing an individual evaluation — not an automated approval that promises medication before anyone reviews your health.
- Cancellation + commitment. Month-to-month, or locked into a 3, 6, or 12-month plan? What are the cancellation terms and fees?
- Honest labeling. Does it clearly state that compounded medication is not FDA-approved and not therapeutically equivalent to the brand? The FTC has fined providers that blurred this.

From 2022 to 2024, semaglutide and tirzepatide were in official FDA shortage, which let pharmacies legally compound low-cost versions (often $150–$300/mo). The FDA declared the tirzepatide shortage resolved in late 2024 and semaglutide in early 2025, and the wind-down grace periods ended around mid-2025. In 2026 you generally cannot get a compounded GLP-1 simply because it is cheaper — compounding is now limited to patient-specific prescriptions with a documented medical reason (an allergy to an inactive ingredient, or a dose not commercially available).
That reshaped the market. Big brand-name platforms Ro and Hims exited compounded GLP-1 and now sell only FDA-approved medication. The FDA issued dozens of warning letters (58 in September 2025, more in 2026) to telehealth firms for marketing compounded products as equivalent to the brand, and the FTC fined NextMed for advertising prices that excluded the medication and for fabricated reviews. On April 30, 2026 the FDA proposed permanently barring bulk 503B compounding of semaglutide, tirzepatide, and liraglutide.
The flip side is better branded access: manufacturer direct-pay (NovoCare, LillyDirect) lowered cash prices, and a Medicare GLP-1 Bridge program brings eligible enrollees a lower copay. Hidden costs to watch for
The sticker price is rarely the whole bill. Before you enroll, check each of these.
- Dose-escalation price jumps
- Many programs price the starter dose low, then raise the monthly cost at higher titration steps. Confirm the price at the maintenance dose, not just week 1.
- Membership fee on top of medication
- Some platforms split the bill: a low medication price plus a separate monthly membership. Add both to get the true monthly cost.
- Required lab work
- A baseline A1C or metabolic panel may be required and billed separately if not bundled. Ask whether labs are included.
- Cancellation friction & lock-ins
- Watch for multi-month commitments, auto-renew after an intro month, and cancel-by-phone-only flows. The intro price is not the renewal price.
- Shipping & cold-chain fees
- Injectable GLP-1s ship cold. Some programs add a shipping or handling fee per order, especially for expedited or summer cold-pack shipping.
