Switching from compounded GLP-1 to branded Wegovy or Zepbound
compounded semaglutide → Wegovy
Jump to section
FDA enforcement is narrowing compounded GLP-1 access. How to transition to branded Wegovy or Zepbound without losing weight-loss progress.
Why consider switching
Compounded semaglutide and tirzepatide became widely available during 2023-2024 brand-name shortages, and many telehealth services offered them at $200-400/month. As Novo Nordisk and Eli Lilly supply normalized, FDA enforcement has narrowed the legal basis for compounding these drugs. Many telehealth services have already transitioned patients to branded; others may do so as policy tightens. Some patients also want the predictability of standardized branded dosing.
What changes
Active ingredient stays the same (semaglutide → semaglutide, or tirzepatide → tirzepatide). What changes: dose precision (branded is FDA-quality controlled, compounded varies by pharmacy), injection device (branded pen vs vial-and-syringe for most compounded), and typically a significant cost increase. Some compounded products included B12 or other additives; branded does not.
Dose conversion
Match your compounded maintenance dose to the equivalent branded step. Compounded 2.5mg semaglutide weekly → Wegovy 2.4mg weekly (closest branded step). Compounded tirzepatide doses map directly to Zepbound's 5, 7.5, 10, 12.5, 15mg steps. Your prescriber will typically start at the matching branded dose, not restart titration, if you've been stable on the compounded dose for ≥8 weeks.
Transition timeline
- Week 0
Last compounded dose; clinician submits branded prescription and prior auth if needed.
- Weeks 1-2
Start branded at equivalent dose. Insurance prior auth typically pends 1-4 weeks.
- Weeks 3-8
Steady state on branded. Some patients report no transition symptoms; others have brief GI uptick.
- Month 3+
Schedule clinical follow-up to confirm weight maintenance and adjust dose if needed.
What to watch for
- No GI re-acclimation if you stay at equivalent dose — but watch for it if your prescriber resets titration
- Cost shock: branded out-of-pocket is often 3-5× compounded
- Insurance prior auth delays — have a 2-4 week supply of compounded as bridge if possible
- Same MTC/MEN2 contraindications apply
Cost impact
Compounded GLP-1s commonly ran $200-400/mo all-in. Branded Wegovy is $1,349/mo list, $499/mo via NovoCare self-pay. Branded Zepbound is $1,060/mo list, $349-549/mo via LillyDirect self-pay vials. Lilly's commercial savings card can drop insured copay significantly. Many patients see 2-3× cost increase even with discounts.
