Saxenda (liraglutide) requires daily injections; Wegovy (semaglutide) is once-weekly with better trial weight loss. How to transition.
Why consider switching
Saxenda (liraglutide 3mg daily) was Novo Nordisk's first weight-loss GLP-1, approved 2014. Wegovy (semaglutide 2.4mg weekly) typically produces more weight loss in trials and is more convenient (weekly vs daily injection). Most clinicians now start patients on Wegovy directly; existing Saxenda patients are commonly transitioned when their plan covers Wegovy or they're willing to pay the difference.
What changes
Active ingredient changes from liraglutide to semaglutide — both GLP-1 agonists, but semaglutide has a longer half-life (enabling weekly dosing) and produces more weight loss at maximum dose. Injection schedule changes from daily to weekly. Side-effect profile is similar; some patients tolerate one better than the other.
Dose conversion
No direct dose equivalence between liraglutide and semaglutide. Standard practice is to discontinue Saxenda and start Wegovy at the lowest dose (0.25mg weekly), titrating up per Wegovy schedule. This restart can cause GI side effects to return briefly even though you were tolerating Saxenda — the body has to re-acclimate to the different molecule.
Transition timeline
- Week 0
Last Saxenda injection; clinician prescribes Wegovy.
- Weeks 1-4
Start Wegovy 0.25mg weekly. Expect some GI side-effect return as you adjust to the new molecule.
- Weeks 5-20
Titrate every 4 weeks toward 2.4mg maintenance dose.
- Month 6+
Steady state. Many patients see additional weight loss on Wegovy versus their Saxenda plateau.
What to watch for
- GI re-acclimation — even though you tolerated Saxenda, Wegovy can cause new nausea during titration
- Same MTC/MEN2 contraindications
- If you're switching due to cost, confirm Wegovy is actually cheaper for your specific plan before transitioning
Cost impact
Saxenda list price (~$1,500/mo) is actually slightly higher than Wegovy (~$1,349/mo). Insurance coverage of Wegovy has expanded faster than Saxenda; many plans that didn't cover Saxenda now cover Wegovy. NovoCare self-pay programs exist for both at similar price points (~$499/mo Wegovy, varies for Saxenda).
