Is compounded semaglutide still legal in 2026?

The legal basis for compounded semaglutide has narrowed significantly. During the 2023-2024 Wegovy shortage, 503A and 503B compounding pharmacies were legally permitted to produce compounded semaglutide under FDA shortage-list provisions. In November 2025, Novo Nordisk announced full Wegovy supply normalization and the FDA removed all Wegovy dose strengths from the shortage list. As of 2026, broad-scale compounding of semaglutide is generally no longer permitted under those provisions. Patient-specific compounding (for documented allergies to commercial excipients, for example) remains legal but is narrowly applied. Many telehealth services that previously offered compounded semaglutide have transitioned patients to branded Wegovy, branded Zepbound, or alternative therapies. The practical impact: cash-pay patients previously paying ~$200-400/mo for compounded now typically face $499/mo NovoCare or $349-549/mo LillyDirect (Zepbound) as the lowest-cost branded options.
